Saul Kaplan: Regeneration, Unleash the Newt Within

photo-saulI have been thinking about regeneration.  While it is common knowledge, it still amazes me, that salamanders can regenerate body parts, including their tails, upper and lower jaws, eyes and hearts.  Yet mammals including humans can’t. Salamanders are the highest order of animals capable of regeneration. Do mammals know something that salamanders don’t? Cosmetic surgery, implants, and promising regenerative medicine research aside we humans are stuck with the body parts we are dealt for now.

I wonder if our inability to regenerate at the biological scale also impedes our ability to regenerate at a social system scale.  It seems obvious that our important social systems including education, health care, and energy need serious regeneration.  These systems have evolved over a long period of time, were built to support an industrial era that is long gone, and have built up incredible mechanisms to resist and prevent needed change.  It is not technology that is getting in the way of social system change.  It is humans and the organizations we live in that are both stubbornly resistant to change. Why are humans so incapable of regeneration at both biological and social scales?

Maybe understanding the biology of regeneration can provide insight.  Salamanders can regenerate injured body parts because evolution has enabled them to immediately unleash stem-like cells to a wound site when damage is detected.  When salamanders are wounded skin, bone, muscle, and blood vessels at the site revert to their undifferentiated state. In essence they go back to an embryonic state and start all over again making regeneration possible.  Humans took a different evolutionary path.

Turns out the human evolutionary pathway traded off regeneration in favor of tumor suppression.  In order to decrease the risk of cancer and increase longevity our mammalian ancestors selected against regeneration.  The theory is rapid cell division required for regeneration looks to our bodies a lot like the unchecked growth of cancer.  Because our longevity makes us vulnerable to accumulated DNA mutations we’ve evolved a kind of molecular brake to keep tumors at bay.  I can’t speak for humankind but it seems like the right trade-off to me.  Unlike salamanders, when mammals lose a limb the body’s reaction is to release cells to the site that become scar tissue.  Current stem cell research is promising and offers the future potential for a work-around to enable regeneration without turning off the molecular brake that prevents tumor formation and progression. Tissue generation and regenerative medicine are both exciting fields to watch.

Beth Gamulka: Books every doctor (and maybe every patient) should read

Beth GamulkaBefore I went to medical school, I spent 4 wonderful years at college studying the history of science and medicine.  I especially loved the books on my reading lists that captured a story about a past medical discovery or epidemic.  While I had no medical expertise at the time and simply wanted to do passably well on the MCAT so some medical school would accept me, the stories of the flu epidemic of 1918, the discovery of penicillin, the Wexner report and the development of formal medical education captivated me.  They gave me context and allowed me to understand the path the practice of medicine had taken prior to my interest in the profession.

Medical school, residency, medical practice and parenthood do not leave much free time for pleasure reading.  While it is still wonderful to escape with a novel, I still gravitate to non-fiction works that focus on medicine.  A few years ago, I gave a lecture to my colleagues about books that every doctor should read.  While it is still prudent to keep updated on new research to help our patients, these books are designed to help physicians (and non-physicians)  maintain perspective.  Here are a few of them:

History of Medicine:

  • The Immortal Life of Henrietta Lacks by Rebecca Skloot is a well-researched account of the history of HeLa cancer cell line and the woman whose cancer launched a medical revolution.
  • Bad Blood by James H. Jones is history of the Tuskeegee Syphilis experiment that the Public Health Service ran from 1932 until 1972. African-American men with syphilis were studied over decades to learn about the natural course of the disease. However, they were never treated with penicillin even after the antibiotic’s availability increased in the 1940s.
  • The Emperor of All Maladies by Siddhartha Mukherjee is a biography of cancer. It is long and detailed and clearly shows how recently many advances in cancer treatment have developed and how little oncologists really knew even 50 years ago.

Meditations on the Profession:

  • Complications by Atul Gawande, a surgeon and prolific writer, is a collection of honest essays written during and after surgical training.
  • The Youngest Science: Notes of a Medicine-Watcher by Lewis Thomas was his third collection of well-written and thoughtful essays reflecting back on his career as a successful academic and physician. I remember reading this in high school and deciding to become a physician.
  • The Real Life of a Pediatrician, edited by Perri Klass is a collection of candid stories following the path from student to veteran doctor. While I love every book that Perri Klass has ever written (and can admit that her memoir A Not Entirely benign Procedure allowed me to survive the summer of 1988, also known as the summer of med school applications and MCATs), the many voices in this collection are honest and engaging. Let’s face it: pediatricians are generally nice people and I like reading about them.

Books that can influence the way we practice:

  • Forgive and Remember: Managing Medical Failure by Charles Bosk, follows fictional surgical teams in a teaching hospital and is one of the first books ever written on medical error.
  • The Checklist Manifesto by Atul Gawande, provides a 21st century approach to patient safety using the expertise of the aviation industry.
  • How Doctors Think by Jerome Groopman is an extremely honest and thoughtful look at medical error and cognitive error. It explores why doctors succeed and why they err, how they can embrace uncertainty and how patients can help doctors avoid error. I tell every trainee that I teach to read this book in the hopes that s/he will incorporate these learning points in the practice of medicine.

Josh Bowen: Is It Really About Fitness? 5 Reasons Why it is Not

joshThe title of the blog may seem confusing and even appalling to most. It may lead you to question my sanity after 11 years as a personal trainer. “How does he not know what his job is?” Just hold on for a minute. For years I have had this approach when it came to clients, focus on more than fitness and nutrition.

See I believe fitness is a vehicle that we use to improve people. People HATE exercise, for the most part. For one reason or another most people disdain the process of trying to get into shape. They have goals but most of the time those goals are deeper than fitness. They impact every aspect of their lives. So, if it deeper than fitness why do we not concentrate on more than fitness? This approach may actually help you jump start your fitness and go on a quest to better yourself.

In my mind exercise/fitness and good nutrition affect every aspect of my life. Therefore, not only do I affect the physical part of me and they way I look but I also affect several other aspects of my life; relationships, better sleep, energy, confidence etc. So if you knew that working out would help boost your confidence and help you have the courage to ask a pretty girl on a date or make a great speech in front of the CEO of you company, would you be more aped to do it? Of course you would. Here are 5 of the most impacted areas of our lives that fitness increases more than anything else.

1.  Confidence

The picture on the left is from my high school graduation in 2000 and the picture on the right is from last year. On the left I weigh an astounding 140lbs on the right 200lbs. Forget the numbers and the obvious physical difference, the number one benefit that fitness has provided me is confidence. Without it, I would not have been able to speak in front of hundreds of people, write a book, go on a bootcamp tour for 14 days across the US or be a personal trainer. Physically I am different but the most effect has been in my confidence.

So many of my clients will list confidence as their number one benefit from working with a trainer and starting an exercise program. The proof is in the pudding.

2. Relationships

As a continuation from confidence, I find that increased relationships with others is a direct reflection from being fit. Often times clients will meet new people and friendships are born or their existing relationships improve because of heightened self confidence.

3. Diligence

A direct reflection of being fit is increasing one’s diligence and determination. Get tasks completed because they have more energy or confidence is common place.

4. Clarity

The hormonal effect of exercise is documented but what you often forget is the increased ability to think clearer. This aspect comes in handy in todays fast moving world. I always have told people I never make decisions until I have worked out that day. Working out will give me the extra time and mental clearness to think about making a decision.

5. Self Esteem

Confidence and self esteem are different. Self esteem is how you view yourself and how you feel about it. Fitness supplies the increased self esteem that one feels like they can conquer anything. Feeling better about the way you look is the best feeling in the world.

All in all, I do not concentrate on fitness. I concentrate on the effects of fitness and how it effects and impacts our lives. Anyone who has started an exercise program an continued with it can tell you that there are more than just five aspects that help better their lives. Focus on the effects working will give you, instead of thinking about whether you can do it or not.

Lauren Mayer: Summertime Blues

Labor Day has traditionally marked the start of the fall season, when we say goodbye to ‘those lazy hazy crazy days of summer,’ return to school (or work), and put away our white shoes until Memorial Day.  Of course, most of those traditions have evaporated – style expert Tim Gunn says white is appropriate all year round, very few working adults get much time off in the summer, and many schools start mid-August or earlier.   But we still usually think of summer as a more carefree time, when things are a little easier and workplaces are more casual.  (I, for one, thoroughly appreciated the break from waking my son up for 4 years of zero period marching band – getting a sleep-deprived teenager out the door at 6:30 a.m., and living to tell the tale, has earned me at least some good karma!)

However, this past summer has been an endless stream of awful news, from war and conflict to corruption trials to racial unrest and protests, and there never seemed to be a lull.  It made me nostalgic for last summer, when the big stories were outrage over ‘twerking’ (Miley Cyrus’ provocative dancing at an awards show), or the continuing revelations in the Anthony Weiner sexting scandal.  Even the Kardashians were surprisingly low-key – I guess they’re waiting to reveal their next big shocker when the world isn’t so fixated on things that actually matter . . .

With that in mind, here’s a salute to the Summer of 2014 – and to how relieved we are that it’s over!

Erica and Matt Chua: Luck Didn’t Get Us Here

“You’re so lucky!” is the worst phrase in the English language.  It’s something we’re told by many people when they find out we’ve traveled for two years without working.  Saying this belittles everyone’s ability to make their dreams come true, even their own, as it wasn’t luck that got us here, it was following the lesson below.  Since this is the week we make resolutions to change, here’s how you can accomplish any goal, no matter how big, as we did when we set off to travel the world.  You can use this guide not to make a resolution, but to make things happen.

Cute?  Certainly, but don’t wait for this little guy to make your dreams come true….

LUCK, LEPRECHAUNS AND LIGHTNING

Before going any further let’s once and for all end the myth of luck.  Luck is like getting struck by lightning, it’s not random, it’s not chance.  If you stand on your roof with a metal poll in a lightning storm, getting struck isn’t “dumb luck”.Not getting struck because you were in your home isn’t luck either.  Lottery winners aren’t any more lucky, rarely has a person hit the jackpot on their first $1 ticket, rather they buy hundreds, if not thousands of dollars on tickets for years before winning.  Getting struck by lightning (or not) and winning the lottery isn’t about luck, it’s about setting yourself up for it.  The reality is luck is as real as the leprechauns who bring it.

CHAMPIONS DON’T GET LUCKY

Michael Jordan, Michael Schumacher or Roger Federer aren’t lucky.  We just weren’t watching them as they perfected their craft for hours every day, for years, before we knew their name.  While we saw them do things in their sport that nobody had done before, we didn’t see the hours each day they worked behind-the-scenes.  Others have had the physical talents of these champions, but only those that put in the work, dedicate themselves to perfection, and overcome obstacles (personal and physical) that become household names.

The same goes for when you watch a professional anything.  What seems so effortless, so easy, is actually the result of years of practice.  Think of something you do that there are professionals, anything from soccer to cooking: don’t professionals make hard things look easy?  Their skills weren’t gained overnight or by a lightning strike, no, they’ve spent years perfecting their craft.

Saul Kaplan: Innovators Leverage the Deadwood

photo-saulIt’s time for me to come clean.  In today’s social media crazed world it will come out sooner or later anyway.  I have one high school varsity letter and it’s for bowling.  Yes, you heard right, bowling.  And it wasn’t ten-pin, but candlepin bowling.  Anyone who grew up in New England, with parents like mine who looked for ways to get the kids out of their hair on rainy Saturdays, knows exactly what I’m talking about.  Candlepin bowling rocks.

For those of you who aren’t from New England, candlepin bowling is a unique version of the sport invented in 1880 in Worcester, Massachusetts by a local bowling alley owner, Justin White.  Candlepin bowling is clearly evidence of New England as a regional innovation hot-spot. For the most part candlepin never caught on outside of New England and the Canadian Maritime provinces.  In the region candlepin bowling enjoyed a cult following including its own local television shows.  I remember Candlepins for Cash, which was a Saturday morning staple and may well have been the first reality television show.

The first noticeable difference from the more popular ten-pin variety of bowling is the small size of the balls.  Don’t look for holes for your fingers because there aren’t any.  The ball is 4 ½ ” in diameter weighing only 1.13 kg.  It fits in the palm of your hand and can literally be thrown rather than rolled down the alley at the pins.  I have seen many errant candlepin balls launched across lanes. Personal injury insurance is a must.  Back in the day I owned a set of balls (spare me the cajones jokes) and yes of course the required bowling ball bag.  The balls were a pearly white with wonderful lime green marble swirls throughout. Come to think of it I wonder where they went.  Most likely my wife sold them at a garage sale when I wasn’t paying attention.

Another difference in candlepin bowling is the size of the skinny pins (15 ¾ ” by 3″) which are harder to knock down so you get three tries in every frame versus the two attempts you get in ten-pin.  My favorite difference in candlepin bowling is that the deadwood between shots isn’t cleared.  In other words pins that are knocked down are left as they lie to either impede or aid the subsequent shot in each frame. You haven’t lived until clearing a 7-10 split which would be all but impossible without the help of well-placed deadwood.  I love this aspect of the sport and in this way candlepin bowling is like the innovation process and life.  There is always deadwood to deal with.  It is how you deal with and leverage the deadwood in your life that defines you.

Josh Bowen: 10 Foods You Need in Your Diet

joshOf all the types of questions a fitness professional is asked, nutrition is the most common subject. Quite frankly people are clueless when it comes to what they eat. Conversely, that is not their fault. In this country we search for the quick fix, the easy way out and our media has done a great marketing job on “diets.” These diets usually consist of restriction some selection of food; fat, carbs, meat etc. So we are constantly told what we “shouldn’t” eat and rarely are we told what we should eat. So I wanted to put together a list of 10 foods that most everyone needs (barring food allergy, religious reasoning or preference). These 10 foods have various purposes that go beyond how many calories they consist of. These foods help the body ward off disease, decrease inflammation, and add vital vitamins and minerals to our body.

1. Eggs- Eggs are on this list for a variety of reasons; the whites are full of high biological value protein (that is a good thing), the yellow is full of leptin (which controls appetite) and they are extremely versatile (scrambled, poached, boiled etc.).  Eggs also have 12 vitamin and minerals, one of which (choline) is vital for brain development and increased memory. Eggs are nutrient dense only containing 75 calories per egg.

2. Nuts- If you only look at foods based upon their fat content and calorie count you would probably leave these section of food out. You would be missing the point of the article. Yes calories matter, however a twinkie may have less calories than a Big Mac that does not mean it is a good option. Most nuts are loaded with Omega 3 fatty acids that are good for the cardiovascular system and act as an anti-inflammatory warding off potential disease. Also nuts are considered an antioxidant which has the potential to decrease the potential for cancer. Because most nuts are high in fat, thus high in calories, use proper judgment and portion control and monitor the salt as well. Almonds, cashews, pistachios are all good choices.

3. Sweet Potato- One of my all-time favorite foods that packs a punch and is nutrient dense. Sweet potatoes are technically a vegetable, although we rarely think of them in that way. Their dark orange coloring makes them high in Vitamin A and C and an outstanding source of potassium. Diabetics can enjoy these foods because they are lower on the glycemic index than white potatoes, generally because of their high water content. Sweet potatoes also act as an anti-inflammatory.

4. Broccoli- Another one of my all-time favorite foods. Without the cheese, broccoli is a true super food that is packed with potassium a rich source of Vitamin A, C and K. It can be very beneficial for diabetics because of its high fiber content. Just remember, no chees

5. Apples- Not all fruits are created equal. Some are good and some are not. Apples however are unique, the skin is full of fiber (blunting the insulin effects of sugar) and they have high antioxidant content. Apples have been linked to prevent lung cancer, improve lung function and help prevent Type II diabetes.

6. Salmon- the American Heart Association recommends eating salmon twice per week because of the high Omega 3 content. It is low in calories, high in protein and low in saturated fats. If at all possible choose fresh salmon versus farm-raised.

7. Kiwis- They are a nutrient dense food that has an entire days requirement of Vitamin C in one large kiwi. Also high in fiber and potassium and a good source of Vitamin A and E. Great taste and texture makes this fruit a great super food.

8. Beans- Beans are loaded with insoluble fiber which is great for ridding the body of unwanted waste. These are heart healthy foods which are low fat good source of protein. Beans are versatile, they can be a good side dish, a great substitute for meat and chicken and can also be mixed in with other foods to make a great snack.

9. Quinoa- What? Yea Quinoa (pronounce keen-wa) is an ancient grain that is now readily available in health food stores and sections of your commercial grocery stores. Its high in protein and fiber and a good source of iron. It is as easy to prepare as rice and can be combined with nuts and beans as a good meal.

10. Berries- pack an incredible amount of nutritional goodness into a small package. They’re loaded with antioxidants, phytonutrients, low in calories, and high in water and fiber to help control blood sugar and keep you full longer. And their flavors satisfy sweets cravings for a fraction of the calories in baked goods. Blueberries lead the pack because they are among the best source of antioxidants and are widely available. Cranberries are also widely available fresh, frozen, or dried. All can add flavor and nutrition to numerous dishes, from salads and cereals to baked goods and yogurt.

So instead of focusing on foods you shouldn’t eat, concentrate on the foods you should. Start incorporating the above and watch your health increase dramatically which will vicariously help you reduce body fat, lower your blood sugar and help you build more muscle. Will Meyerhofer: Save the World

Will MeyerhoferIf law students are annoying, then pre-law students are twice as annoying. There’s something about observing these lemmings scrabble their way into the maws of ruthless law schools, despite dire warnings and appeals to common sense, that just…gets under my skin.

Even after so much effort has been expended for their benefit – i.e., which part of “Way Worse Than Being a Dentist” didn’t you understand? – these piteous creatures patiently queue up for their punishment, hungry to “learn to think like a lawyer.” If your resolve weakens, and pity prevails over contempt, you might mistakenly engage one in conversation. For your trouble, you’ll receive an earful of a clueless pipsqueak’s master plan to save the world. Because – you hadn’t heard? – that’s why he’s going to law school: The betterment of humanity.

Because that’s what the world so desperately needs:  Another lawyer.

Somehow or other, these automata get it into their programming that, if they actually did want to save the world, becoming a lawyer would be a sensible way to do it. They are unaware of how imbecilic their words sound to anyone not entirely befuddled by the miasma of law school propaganda.

Law schools inundate proto-lawyers with ‘lawyers save the world’ nonsense, cramming their crania with musty tales of Brown v Board of Ed. That’s because the schools are well aware of the likely effect of such indoctrination: Greasing the rails to the killing floor. If a kid can tell himself he’s going to “change the world” – as opposed to, say, “make a lot of money and feel like a big deal” – then he’ll line up that extra bit more smugly for the $160k/year that makes his eyes roll up into his head and a little string of drool form at the corner of his mouth.

It’s simple: If you can tell yourself you’re doing it for the good of humankind, you won’t feel so guilty selling out in the most soulless, stereotypical way imaginable.

 

You know the vast majority of law students will end up deeply in debt and unemployed. We all know that. But before that happens, the sorry little shlemiels honest-to-god tell themselves they’re going to save the world.

The problem is lawyers very seldom do change the world, at least for the better. The bulk of significant positive change that the world experiences at any given moment – surprise! – doesn’t derive from the actions of lawyers. It derives from the actions of non-lawyers, or, at very least, lawyers acting in non-lawyer-y ways.

Evidence? Let’s start with a quote from one of the nation’s top civil rights attorneys, Michelle Alexander, from her book, The New Jim Crow: Mass Incarceration in the Age of Colorblindness:

In recent years…a bit of mythology has sprung up regarding the centrality of litigation to racial justice struggles. The success of the brilliant legal crusade that led to Brown has created a widespread perception that civil rights lawyers are the most important players in racial justice advocacy…Not surprisingly…many civil rights organizations became top-heavy with lawyers. This development enhanced their ability to wage legal battles but impeded their ability to acknowledge or respond to the emergence of a new caste system. Lawyers have a tendency to identify and concentrate on problems they know how to solve – i.e., problems that can be solved through litigation. The mass incarceration of people of color is not that kind of problem.

Got that? Here’s a top-flight lawyer, at the center of a struggle to address the disaster of a nation that locks up a vast percentage of its poorest, most vulnerable citizens based largely on their race (whites don’t go to jail for minor drug possession offenses, blacks do.) What’s she saying? There are too many lawyers.
